Individualized Healthcare Plan Project

Heartland’s Individualized Healthcare Plan (IHP) Project is on its implementation phase! We are looking for up to 20 participating schools/school nurses to attend the training for implementation on August 8-9, 2016 in Kansas City, MO. Heartland will cover all travel-related expenses.
