The Heartland Genetics Services Collaborative is partnering with the Region 4 Midwest Genetics Collaborative to provide Care Coordination: Empowering Families training in the Heartland region. The purpose of the training is to provide parents with the skills, knowledge, and resources they need to coordinate care for children with genetic condition in partnership with a medical home.
