The Role of the Geneticist in the Medical Home

Although much has been published about the patient-centered medical home, the role of the medical geneticist within this model of care has not been delineated in the literature. After reviewing the literature, debating practice models, and communicating with colleagues in primary care as well as geneticists across the Heartland region, the ad hoc Heartland Medical Home Work Group documented their position in a paper now available on the on our website (full version). A shorter version in the form of a commentary has also been submitted for publication. Building on this work, a genetic counseling graduate student has explored the perspectives of medical geneticists, parents, and primary care providers about the role of the medical geneticist in the medical home. These works will be presented at the Heartland 11th Annual Conference on April 29 – May 1, 2015.
