The Heartland Genetics Services Collaborative is partnering with the Region 4 Midwest Genetics Collaborative to provide Care Coordination: Empowering Families training in the Heartland region. The Care Coordination: Empowering Families training curriculum was developed by the Region 4 Midwest Genetics Collaborative in partnership with public health, genetic and primary care providers, and family members. The purpose of the training is to provide parents with the skills, knowledge, and resources they need to coordinate care for children with genetic conditions in partnership with a medical home.
Heartland aims to assist partnering organizations and their selected facilitators to deliver effective training using the Care Coordination: Empowering Families curriculum. Travel and technical assistance will be provided for up to 16 organizations to become certified trainers of Care Coordination: Empowering Families. Organizations that provide training and outreach to families will be considered strong candidates for partnership in the Care Coordiation: Empowering Families and facilitator certification.
